Why Senior Care Record Map exists
Most senior-care directories are lead-generation businesses: they're paid by the facilities, so the "recommendations" are really paid placement, and the records that matter most — inspection deficiencies, substantiated complaints, probation, abuse citations, CMS special-focus status — are buried or missing. Senior Care Record Map takes the opposite stance. We take zero referral fees. You're our customer, not the facilities. That's why we can rank places by their government safety record and show you the warnings the lead-gen sites hide.
Where the data comes from
Every grade is built from public records:
- Assisted living & residential care — official state licensing registries. Coverage and inspection detail vary by state.
- Nursing homes (nationwide) — the federal Centers for Medicare & Medicaid Services (CMS Care Compare / Medicare.gov), including star ratings, health-inspection deficiencies, and staffing.
We currently cover more than 42,000 assisted-living, residential-care, and Medicare-certified nursing-home records across all 50 states, DC, and territories. Each facility page identifies its source and, where comparable inspection data is available, explains the safety grade.
