Report an error or dispute a record
If something on a Senior Care Record Map facility page is wrong, out of date, or missing context, tell us and we'll fix it. Facility operators can also submit an official response that will appear on their page.
What to send
The fastest way to get a correction is to include:
- The facility name and the Senior Care Record Map page URL (or facility ID).
- Exactly which field or claim you believe is wrong.
- The correct value and, if possible, the public source that supports it — a state licensing profile, an inspection report, a CMS record, or a dated screenshot of the official regulator page.
- Your role, if relevant (facility administrator, owner, family member, resident, surveyor). We do not publish this unless you ask us to.
Submit a report
Facility responses published on the page
Any licensed operator can submit a short, factual response that Senior Care Record Map will publish directly on their facility's page under a clearly labeled Facility response section. There is no charge, no advertising component, and no editorial rewriting of your substance. We reserve the right to trim for length, remove personal information about residents, and ask for supporting documentation before publishing anything that contradicts a government record.
Guidelines:
- Keep it to ~200 words and stick to facts a reader can verify.
- Explain context that the raw inspection or complaint count doesn't capture (corrections completed, ownership change, staffing improvement, etc.).
- Don't identify residents, staff, or complainants by name.
- Include a contact at the facility so we can reach you if a reader disputes the response.
Once a response is published, we keep it in place as long as it remains factually consistent with the current regulatory record. We date-stamp it and will attach an update if the record changes.
Listing removal
Senior Care Record Map indexes public government records about licensed senior-care facilities. Because those records are already public, we generally do not remove a listing at a facility's request. We will:
- Remove a listing that is no longer licensed or has closed — send us the dated closure or license-revocation record and we'll retire the page (with a redirect for search engines).
- Remove personally identifying information about residents, staff, or complainants if it appears anywhere on the page.
- Correct a listing that is a duplicate or that mixes records from two facilities.
If you believe a listing violates a specific law that requires removal (defamation with proof of falsity, a court order, GDPR right-to-erasure for an EU resident, DMCA copyright claim, etc.), state the legal basis in your report and include supporting documentation. We take those seriously and respond promptly.
Safety-grade disputes
Every Senior Care Record Map safety grade lists the public records behind it — visits, citations, complaints, enforcement actions — with their dates and links back to the state or federal regulator. If you believe a grade is wrong:
- Check the record on the regulator's own site (linked from the Senior Care Record Map page). If it's wrong there, you'll need to dispute it with the regulator first — Senior Care Record Map cannot override the government's own record.
- If the government record has been updated or corrected since our last snapshot, send us the new source and we'll re-pull.
- If you believe Senior Care Record Map has mis-attributed a record (wrong facility, wrong license number, out-of-window inspection), tell us which record and we'll investigate.
- If you disagree with the weighting our model applies, we're happy to publish a facility response explaining your view. The methodology is disclosed on the grade guide.
Privacy of your report
Reports come to us by email. We do not publish, sell, or share your contact information. Aggregate patterns (e.g. "12 reports about missing capacity fields in Oregon") may inform how we improve the data pipeline. See our privacy policy.
